We’re making updates to Young Life’s U.S. Health and Consent Forms. The online 6007 Health and Consent form will not be available until early 2026. When the new Health Form launches, the previous paper 6007 will not be considered valid. For summer 2026, if areas bring an old version to camp, parents will need to complete an updated form before their child can participate in any activities. Please stop using, distributing, or saving any older versions of the form to avoid confusion.
